The Stepped Insufflation On/Off Indicator (12) will show orange.
The Service connections (25 & 26) on the rear of the panel are used during manufacture, calibration and servicing of the LINS-2000.
13
5 STEPS FOR PATIENT APPLICATION Please complete only if the initial set-up procedure (Section 4) has been completed.
5.1 Veress needle insertion
1. Ensure that the tubing is correctly attached to the insufflation Gas Line Connection (10) and to the Veress needle. 2. For verification of correct Veress needle position, the negative pressure test can be performed at this stage and the actual Intra-Abdominal Pressure display (6) will display a negative value up to 9mmHg.
NOTE:
It is difficult (for all insufflators) to deliver gas through most Veress needles at a rate greater than 6 L/min. If flow rates greater that 6 L/min are attempted through a Veress needle, the LINS –2000 may display an “E13” error message. Reduce the selected flow rate to less than 6 L/min before restarting the insufflator.
5.2 Start Procedure Stepped insufflation mode
The LINS-2000 has an automatic stepped insufflation mode. This provides safe, low-flow to begin the insufflation. The low flow rate then automatically increases, in steps, to the maximum set flow rate. This stepped mode occurs in three stages: 1. On pressing the Insufflation Start/Stop Touch-pad the flow rate begins at 1 L/min with ten one –second pulses of gas. Intra-abdominal pressure measurements are taken between each pulse. 2. While remaining at 1 L/min, ten two-second pulses are then delivered with pressure measurement between each pulse. 3. If no blockage is encountered, the flow rate steps up to half the set flow rate as shown on the Set Flow Display (17). 4. Finally, the flow increases to the set flow rate as shown on the Set Flow Display (17). This flow rate continues with two-second pulses until the intra-abdominal pressure reaches the set Intra-Abdominal Pressure (15).
NOTE:
USE7787v4
The LINS-2000 has been calibrated so that the selected flow, shown on the Set Flow Display (17), is that delivered through a 2.0mm ID Luer fitting, common to most laparoscopic ports. The actual flow rate, shown on the display, may differ if other fittings are preset attached (such as a Veress Needle).
14
5 STEPS FOR PATIENT APPLICATION 5.3 Start Procedure Non Stepped Mode
If the stepped insufflation mode is not required, depress the Stepped Insufflation Override Touch-pad (13) and the LINS-2000 will proceed to full flow until the set pressure is achieved. The Stepped Insufflation Mode can be re-commenced by depressing the Stepped Insufflation Override Touch-pad (13) again. The LINS-2000 will automatically default to the stepped mode if the unit is turned off and re-started.
5.4 Maintenance of Pressure
1. The LINS-2000 measures the intra-abdominal pressure every two seconds during its operation. Insufflation only occurs when the intra-abdominal pressure display is lower than the set pressure display. 2. The LINS-2000 automatically recompensates for gas loss at the set gas flow rate. To reduce the possibility of high intra-abdominal pressure, ie (greater than 20mmHg) the duration of gas flow is adjusted dependent on the difference between set intra-abdominal pressure and actual intra-abdominal pressure 3. The set flow and the set pressure parameters can be adjusted at any time during operation using the appropriate up and down touch-pads (16 & 18).
5.5 Standby Mode
If required the unit can be put into standby mode by pressing the Insufflation Start/Stop Touch-pad. If the unit is put into standby mode the start procedure can be reinitiated by pressing the Insufflation Start/Stop Touch-pad (14). Flow will recommence until the set intraabdominal pressure is reached.
5.6 Gas Volume Reset
The Gas Volume delivered display (8) will show a maximum of 999 litres. To reset the display to zero the volume delivered Reset Touch-pad (7) should be pressed. The unit automatically resets the display to zero whenever the LINS-2000 is turned on.
USE7787v4
15
6 ALARM CONDITIONS The Lins-2000 has both audible and visual alarms to alert the users to a variety of conditions. 6.1 High Intra-Abdominal Pressure
If the actual intra-abdominal pressure is 5mmHg above the maximum set pressure and remains at this level for five seconds, the unit will alarm with a multiple pulse tone until the actual intra-abdominal pressure drops to a level less than 5mmHg above the maximum set pressure. While alarm tone is on, the intra-abdominal pressure display will flash.
6.2 Gas Supply
A multiple pulse audible alarm will sound in specific situations: 1. When one gas cylinder empties and the LINS-2000 automatically transfers to the other full cylinder (green indicator), the unit alarms once only. 2. When one cylinder is empty (red indicator) and the second cylinder goes from green to yellow, unit will alarm every two seconds until empty cylinder is replaced with a full cylinder. 3. When one cylinder is already empty (red indicator) and the other cylinder goes from yellow to red, the unit goes to standby mode and the alarm stops.
6.3 Internal Heater
If the internal heater develops a fault, a continuous tone audible alarm will sound without a visual message.
6.4 Block Alarm
If a blockage is encountered during a procedure, the unit will stop insufflation. The gas volume delivered display will flash the letters “blc”, the Insufflation Status Indicator (11) will turn orange and an audible alarm will sound for five seconds. The unit will then return to stand-by mode. A blockage or obstruction is considered to have occurred if intra-abdominal pressure rises more than 10mmHg from one pressure measurement to the next. The unit only alarms for blockage when it occurs during stepped insufflation or the first two gas deliveries of nonstepped mode. If blockages occur later in the procedure, the high pressure alarm will be triggered instead.
6.5 Patient Heated Line
If a continuous audible tone lasting longer than 5 seconds is heard, this may indicate a temporary fault condition has occurred within the LINS-2000 unit. This audible tone does not prevent the unit from operating safely, but usually indicates that no current is being applied to the heating wire contained within the heated gas line being used. Turning the unit off and then back on again should stop the audible alarm and reset the unit so that current is once again being supplied to the heated gas line. In the event that the audible alarm continues after turning the unit back on again, the unit should be examined by a qualified IntelliDesign Service Engineer.
