Important general notes and instructions Make sure that this product is used only as intended and described in this instruction manual, by adequately trained and qualified medical personnel, Maintenance and repair must be carried out by authorized experts. Use the product only in the combinations and with the accessories and spare parts specified in this instruction manual. Use other combinations, accessories and replacement parts only if they are expressly intended for the planned appli cation and if the performance characteristics and safety requirements are met. The product must not be altered in any way. Reprocess the product in accordance with the manual before every use and before return shipment to protect the pa tient, user and third parties. This manual is an integral part of the product and must be stored in such a way that it is accessible at any time during its entire life cycle. This manual must be passed on to any subsequent owner or user. Immediately on receipt, check the product and its accessories for completeness and possible damage. Should the shipment give rise to complaints, please inform the manufacturer or supplier immediately. Subject to technical changes! Technical description PANOVIEW Operating telescope "Piranha" Scope, in the following called PANOVIEW operating telescope with sheath tube, integrated light guide, optical system, working channel and 2 irrigation channels with cold-light connector with adapter (light cable to endoscope) (can be unscrewed) with offset parallel view and eyepiece with eyecup with automatic sealing element with irrigation ring, removable with irrigation stopcock Reprocessing basket with special holders and retaining hooks with locking mechanism with utensils basket
IMPORTANT! The access instruments listed in the following as necessary for the intended use are described in GA-D345 and BB-D345. These manuals must also be observed. Intended use PANOVIEW operating telescope is used for visualizing the inside of the patient via natural passages. houses the automatic sealing element and the irrigation stopcock. is used for gas-tight insertion of the morcellation probe via the sealing ele ment through the working channel into the operating field under visual con trol. is used for continuous irrigation fluid supply via two integrated irrigation chan nels. Under endoscopic control, the PANOVIEW operating telescope is used as follows: The PANOVIEW operating telescope with the corresponding sheaths and ob turators in conjunction with a morcellation probe is used for disintegration (morcellation) for continuous removal of large portions of tissue PANOVIEW operating telescope is used as nephroscope with the corresponding auxiliary instruments for disintegrating and removing / suctioning off kidney and bladder stones for removing tumors via percutaneous (kidney) or transurethral (bladder) passages, in conjunction with intracorporeal lithotripters, e.g.: Pneumatic Ultrasound Electrohydraulic Lasers

Reprocessing basket is used for safe and gentle housing of the PANOVIEW operating telescope and endoscopic accessories during transport/shipment reprocessing sterilization storage These products are exclusively intended for use by medical staff and may only be used by medically qualified and adequately trained doctors.

Indications and field of use

Contraindications and side effects
4.1
Contraindications
The product is used for therapy with transurethral access, in the case of diseases of the lower urinary tract e.g. laser TURP (TransUrethral Resection of the Pro state). As a nephroscope it is used for diagnosis and therapy of urolithiasis and tumors in the upper and lower urogenital tracts. In both cases it is used with endoscopic accessories.
CJD - Creutzfeldt Jakob Disease or a vCJD - Variant of the Creutzfeldt-Jakob Disease BSE - Bovine Spongiform Encephalopathy; so-called mad cow disease (e.g. Creutzfeldt Jakob Disease) TSE - Transmissible Spongiform Encephalopathy On the basis of the patient's general condition the doctor in charge must decide whether the planned use is possible or not. Comply with the laws and regulations valid in your country. For further notes and instructions please refer to the latest medical literature. Contraindications directly related to the product are presently unknown.

Side effects Injuries of the patient, user or others as a result of incorrect combination of products are possible. Toxic or inflammatory tissue reactions are possible in the case of insufficient cleaning, insufficient product reprocessing, corrosion or loss of particles. Tissue damage or burns due to extreme heat as a result of high light energy, soiling or wear are possible. Extreme heating or ignition of heat-sensitive, flammable surfaces as a result of extreme temperatures at the light exit area is possible. Injuries or perforations may occur due to excessively fast and incorrect forward movement of the auxiliary instruments or when working outside the field of view.

CAUTION! Do not combine products incorrectly! Injuries of the patient, user or others as well as damage to the product are pos sible. The different products may only be combined if the intended use and the relevant technical data (working length, diameter, etc.) are the same. Follow the instruction manuals of the products used in conjunction with this prod uct. Fig. 1

The light cable to endoscope adapter (1.9) can be unscrewed and replaced by suitable adapters to connect fiber light cables made by other manufacturers. For order data please refer to the latest catalog sheets.

Use CAUTION! The products have only limited strength! Applying excessive force will lead to damage, impair the function and endanger the patient. Immediately before and after each use, check the products for damage, loose parts and completeness. Make sure that no missing parts remain in the patient. Do not use the products if they are damaged or incomplete or have loose parts.

Initial preparation Check assembly: see section 9.3 Carry out a visual check: see sections 8 and 8.1 Fig. 3 Screw on the light cable to endoscope adapter (1.9).

Unscrew the sealing element (1.13.2) and check whether it contains a sealing membrane (1.13.1). Firmly tighten the sealing element (1.13.2). Install the sealing cap (1.13.3).

Connecting the PANOVIEW telescope (1) to the system components Connect one end of the fiber light cable to the cold light connector and the other end to a suitable light projector. Fig. 6

IMPORTANT! To achieve optimum light transmission, the fiber bundle diameters of the endoscope and fiber light cable must be matched to each other. Possible consequences if the identification does not match: Fiber light cable with excessively large fiber bundle diameter (cross-section): excessive heating at the coupling point with the endoscope Fiber light cable with excessively small fiber bundle diameter (cross-section): reduced light output The color coding ring and the code number on the endoscope identify the suitable fiber bundle diameter. The color coding ring and the code number on the fiber light cable, light cable to endoscope, must be the same.
CAUTION! Intense heat due to high light energy! Under unfavorable conditions, the use of the high-temperature resistant fiber light cable 8065xxxx can cause a temperature increase at the coupling between the fiber light cable and the endoscope. The user may suffer burns and the endoscope may become damaged. Reduce the light ouput, use fiber light cable with product number 8066xxxx / 8062.xxx if necessary. Carry out a function check: see section 8.2.1

Additional notes and instructions for use IMPORTANT! Use only products with type BF or CF applied parts in conjunction with the PAN OVIEW operating telescope (1).
7.2.1
Connecting the continuous irrigation outer sheath (2) and PANOVIEW operating telescope (1) to the suction and irrigation system Check the suction and irrigation function of the preassembled instrument set as well as the entire system for leak-tightness and patency (free passage) before each use. Connect the irrigation tubes to the luer fitting (1.6.4) of the irrigation ring as sembly (1.6) and continuous irrigation outer sheath (2) and hook up to the irrigation system. Open the stopcock plugs (1.6.1) (d). Keep the stopcock plug on the irrigation connector (1.15) closed during the entire procedure. Switch on the irrigation system and check the irrigation function.

Irrigation fluid
CAUTION! Liquid may be pressed into the patient's body as a result of overpressure! This will cause severe injury to the patient. To ensure a low-pressure continuous irrigation effect, suctioning must be effected via the morcellator. CAUTION! Danger of elevated temperatures when working without irrigation fluid! Injuries of the mucous membrane due to excessive temperatures endanger the patient. Activate the laser fiber / morcellator probe only in irrigation fluid and under con tinuous irrigation. 7.2.3
Atraumatic insertion of resectoscope sheath If the resectoscope sheath is not in place yet, insert the latter atraumatically.
CAUTION! Do not insert the resectoscope sheath without obturator! This may cause inadvertent tissue damage. Insert the resectoscope sheath only atraumatically with the obturator in place. Insert the obturator into the resectoscope sheath and introduce into the urethra. Remove the continuous-irrigation inner sheath with obturator.

7.2.4
Light
WARNING! Intense heat due to high light energy! Danger of inadvertent tissue damage due to insufficient distance between the light exit area and the tissue due to soiling / contamination in the light exit area when using high performance light projectors. Do not touch the light exit area and avoid direct contact with the tissue. Remove any soiling or contamination. WARNING! Fire hazard! Extreme heat at the light exit area can cause excessive heating or even ignition if the endoscope is placed against heat-sensitive flammable surfaces (dark drapes, etc.) Store the endoscope in a safe place. Switch off the light source if you do not use the endoscope for a period of time. CAUTION! Danger of burns! After disconnection from the light projector, the adapter and glass surface of the fiber light cable are extremely hot as a result of the high light energy of the light source. Burns may result from unintentional contact. Do not touch the adapter and glass surface. Allow the components to cool down. 7.2.5
Electrical currents
WARNING! Danger from electric shock! Patient leakage currents can add up if the endoscopes are combined with other powered endoscopic accessories. Make sure that the combinations do not exceed the permissible patient leakage currents. 7.2.6
Image quality
CAUTION! Increased risk potential if the image is blurred! Danger of injuring the patient. Stop the intervention for safety reasons if the image is blurred. Check the image quality of the endoscope before use (section 8.2.1).
7.3
Laser application
When applying a laser make sure you follow the laser device manufacturer's in structions as well as the general regulations on the use of lasers. Wear the required personal protection equipment.
CAUTION! Do not work outside the scope's field of view! Inadvertant tissue damage as well as damage to the distal end of the PANOVIEW telescope (1) and instrument parts is possible. Activate the laser only after the tip of the laser has become fully visible through the PANOVIEW operating telescope (1) and contact is made with the intended treatment area by means of the pilot beam.

CAUTION! High temperatures due to highly coherent laser beam! The heat generated by the laser beam reduces the strength of instrument parts. Do not direct the laser beam at instrument parts, in particular not at plastic parts. Keep a safe distance. CAUTION! Highly coherent laser beam! The laser energy may cause injuries in the the user's face around the eyes. Wear the required personal protection gear when using this product in conjunc tion with laser devices. An additional suitable filter attachment is required when using lasers and working with endoscopes under direct view. CAUTION! There is a risk of eye injuries when working without filter attachment! Use a suitable filter attachment on the eyepiece of the PANOVIEW operating telescope (1).
7.4
Bladder syringe, evacuator A bladder syringe or evacuator can only be used in conjunction with the resecto scope continuous-irrigation double-sheath system consisting of a continuous-irriga tion outer and continuous-irrigation innner sheath.
IMPORTANT! In addition, follow the manual “Syringes, evacuator”, order no.: GA-D 096.

Tissue morcellation IMPORTANT! Follow the morcellator manual! IMPORTANT! To prevent the bladder from collapsing during morcellation, we recommend using the irrigation connector on the continuous-irrigation outer sheath (2) as an addi tional drain to allow for the connection of a second irrigation bottle/bag. Evacuation is effected via the morcellator. The proximal irrigation connector (1.15) remains closed.
7.5.1
Carrying out an instrument change
NOTE! When changing the instruments from laser enucleation to tissue morcellation, we recommend the following procedure: Laser resectoscope sheath with working element and PANOVIEW telescope are in situ. Disassembling the laser resectoscope Remove the laser fiber. Close the drain stopcock on the continuous-irrigation outer sheath. Close the irrigation stopcock on the continuous-irrigation inner sheath. Switch off the irrigation fluid supply. Remove the components in the following order: Supply and drain tubes Fiber light cable Camera head and objective lens
IMPORTANT! Do not remove the continuous-irrigation inner sheath with working element and PANOVIEW telescope from the outer sheath remaining in situ.

Preparing the PANOVIEW operating telescope (1) Connect the components listed in the following order to the PANOVIEW oper ating telescope (1): Supply tube Fiber light cable: see section 7.1.2 Camera head and objective lens Set the white balance. Changing the instrument Remove the continuous-irrigation inner sheath together with the working ele ment and PANOVIEW telescope. Insert the preassembled PANOVIEW operating telescope (1) under visual con trol into the continuous-irrigation outer sheath (2): see section 7.1.1 The locking mechanism engages automatically. Switch on the irrigation fluid supply. Open the irrigation stopcock. Introduce the morcellation probe proximally through the automatic sealing valve (1.13).
NOTE! Advantage of the quick instrument change described above: The bladder cannot collapse. The prostate won't start bleeding again. This guarantees good visibility and orientation.
7.6
Nephroscopy
In conjunction with an Amplatz sheath (26 Fr.), the PANOVIEW operating telescope can be used as a nephroscope. For nephroscopy, puncture the hollow organ with a conventional puncture set and dilate to the required width (if not carried out before). Slide the Amplatz sheath over the dilator into the pyelocaliceal system. Remove the dilator and insert the PANOVIEW operating telescope (1). Carry out this procedure under X-ray control.

Checks CAUTION! Be careful if products are damaged or incomplete! Injuries of the patient, user or others are possible. Run through the checks before and after each use. Do not use the products if they are damaged, incomplete or have loose parts. Return damaged products together with any loose parts for repair. Do not attempt to do any repairs yourself.

Check the products and the accessories for: damage sharp edges loose or missing parts rough surfaces Any lettering, labeling or identification necessary for the safe intended use must be legible. Missing or illegible inscriptions and labels that can lead to wrong handling and reprocessing must be reinstated. 1.13.1
Fig. 8 Check the O-rings (1.5), sealing cap (1.13.3) and sealing membrane (1.13.1) for damage. Replace brittle and cracked O-rings (1.5), sealing cap (1.13.3) and sealing membrane (1.13.1).

Function check Check the individual components for compatibility. Check for secure connections. Check for easy assembly and proper functioning of the locking mechanism of the individual products. Replace the products if the connection although locked is not secure. cannot be locked or is difficult to lock. Check easy insertion of auxiliary instruments. Check rotatability of the PANOVIEW operating telescope (1) in conjunction with the continuous-irrigation outer sheath (2). Check the irrigation and suction functions. Check the entire system for leak-tightness and free passage (patency).

8.2.1
PANOVIEW operating telescope (1) Check image quality and light output in conjunction with the system compon ents. Fig. 9 Check the glass surfaces for any deposits. Deposits on the glass surfaces can cause a spotted or blurred field of view and hence impair light transmission considerably. Wipe the glass surfaces with a swab soaked with alcohol (wooden swab car rier, not metal or plastic), remove hard-to-remove deposits with instrument cleaner*.

Stopcock plug (1.6.1) Check that the stopcock plug (1.6.1) is securely locked in the stopcock housing (1.6.2). Fig. 11 Check the stopcocks for leak-tightness. Connect the supply tube, turn the stopcock plug (1.6.1) to its locked position. In case of leaking stopcocks: Replace the stopcock plug (1.6.1). Check that the stopcock plug (1.6.1) moves easily in the stopcock housing (1.6.2).

Reprocessing and maintenance WARNING! Creutzfeldt-Jakob Disease! If the patient is suspected of having Creutzfeldt-Jakob Disease (CJD) or a variant of the Creutzfeldt-Jakob Disease (vCJD) or the latter have been diagnosed, ad equate measures must be taken to prevent possible transmission to other patients, users and third parties.

Disassembly before cleaning
Remove the morcellation probe. Remove all connections between the PANOVIEW operating telescope (1) and the system components. Remove the PANOVIEW operating telescope (1) and the continuous-irrigation outer sheath (2).

Automatic sealing element (1.13) Fig. 13 Remove the sealing cap (1.13.3). Unscrew the sealing element (1.13.2) and remove the sealing membrane (1.13.1).
