Information on the various models of Riester sphygmomanometers You have just purchased a high-quality Riester precision sphygmomanometer manufactured in conformance with European standard EN 1060, Part 1: "Non-invasive sphygmomanometers General Requirements" and Part 2: "Supplementary requirements for mechanical sphygmomanometers" and subjected to constant stringent quality control. The outstanding quality of this instrument will guarantee you years of reliable blood pressure measurement. All Riester mercurial sphygmomanometers come with a mercury lock to keep mercury from running out. All Riester mercurial sphygmomanometers come with glass tubes with an especially thick inside diameter of 4.2 mm +/- 0.2 mm (the walls of the glass tube used in the standard nova-presameter® model are even 5.0 mm +/-0. 2 mm thick), which ensures outstanding readability of the measured values.

Open the case by pressing the button on the front of the case and raising the lid with the graduated scale until it snaps into vertical position. Place the unit in a horizontal position when measuring. Mercury lock function: These models come with a mercury lock in the form of a lever. The unit is delivered with the lever flipped down to the right, i.e. the mercury lock is closed (see Fig. 1). Before using the unit, you must flip the lever to the left until it engages so that the mercury can rise from the reservoir to the zero mark on the scale. Connecting the 2-tube cuffs. Insert (diplomat-presameter® models) or screw (nova-presameter®) the two connectors at the ends of the cuff tubes into the two tube receptacles on the manometer.

Mercury lock function: This model contains a mercury safety button. When shipped, this rotary button is turned to the left "off" position, i.e. the mercury lock is closed (see Fig. 6). Before using the device, this rotary button must be turned to the right up to the stop of the "on" position so that the mercury can rise from the reservoir to the zero mark on the scale.

Connecting the 2-tube cuffs: Screw the tube connector of the spiral tube to the tube connector of the device. Screw the tube connector of the cuff tube to that of the spiral tube.

The desk models must be placed in a horizontal position for measuring. 3.2 Mercury lock function: These models contain a mercury safety button. Now screw together the tube connector on the end of the cuff tube and the tube connector on the coiled tube. 3.3.b. empire® N wall, stand, and anaesthetic models: See "3.3.a". The tube connector is, however, located at the bottom of the device. 3.3.c. global de luxe® Wall Model: Push the spiral tube onto the tube connector at the bottom of the device. Moisten the tube with some water for easier fitting. Next, screw the tube connector of the cuff tube to that of the spiral tube.
Installation instructions for the different models (apart from desk models) Wall models A. empire® N wall model Undo the wing nut underneath the cuff basket and remove the wall mounting. Place the wall mounting against the wall at the desired location. Mark the points to be drilled and drill holes. Insert wall plugs into holes. The wall mounting can now be secured (see Fig. A). Place the device onto the wall mounting in such a way that the top part of the wall mounting fits into the rim of the cuff basket and the bottom part fits onto the bolt protruding from underneath the cuff basket of the device. Next, refasten the wing nut on the protruding bolt. B. nova-presameter® wall model Place the device against the wall at the desired location and mark the points to be drilled. Drill the holes and insert the wall plugs. Secure the device with screws. C. global de luxe® wall model Undo the bolt at the rear of the housing and remove the rail. Place the rail against the wall at the desired location and mark the points to be drilled. Drill the holes and insert the wall plugs. After screwing the rail to the wall, first insert the device at the bottom part of the rail and then secure it at the top part with screws. Please note that wall models have to be installed in a vertical position! Stand models Insert the pole into the hole provided for this purpose in the base unit with 5 rollers. Fasten it to the base unit by tightening the hexagon socket screws with the enclosed Allen key.

empire® N A threaded guide pin is supplied with the device. Apply some adhesive onto the thread, insert the pin into the opening provided underneath the device and secure by turning (see B). Place the device with the pin onto the tube of the stand and, using the fixing screw, secure it at the desired height. nova-presameter® stand model Insert the guide pin up to the stop into the opening underneath of the device. Tighten the pre-assembled bolt on the pin with a screw driver. Next, insert the sphygmomanometer with the pin into the tube of the stand and, using the fixing screw, secure it at the desired height.
Ensure that blood pressure manometers are horizontal before use. empire® N anaesthetic model Remove the wing nuts below the cuff basket and remove the wall mounting. Using the supplied bolts, secure the wall mounting to the back of the universal clamp no. 10384 (see C). Place the device onto the wall mounting in such a way that the top part of the wall mounting fits into the rim of the cuff basket and the bottom part fits onto the bolt protruding from below the cuff basket of the device. Next, refasten the wing nut on the protruding bolt.
Selection of suitable cuff sizes A.
Nylon-velcro cuffs Our nylon-velcro cuffs have a tufted strip on one side and hooks on the other. This ensures that the cuffs can be easily opened and closed quickly and repeatedly. On all the models the cuffs are calibrated, i.e. provided with measurement lines. To make sure that you have chosen the right cuff size, check to see whether the white index line is in the range between the arrows after the cuff has been put on. If the index line fails to reach this range, the cuff is too small. If it is beyond the range, the cuff is too large. Precise blood pressure readings can be obtained only if the correct cuff size is used. The following cuff sizes are available for all models: Newborn arm circumference: 5.0 - 7.5 cm Infant arm circumference: 7.5 - 13 cm Child arm circumference: 13 - 20 cm Small adult (thin arms) arm circumference: 17 - 26 cm Adult arm circumference: 24 - 32 cm Large adult (heavy arms) arm circumference: 32 - 42 cm Thigh leg circumference: 42 - 50 cm Thigh XL (heavy thighs) leg circumference: 50 - 70 cm B.
Cotton bandage cuffs There is a bandage strip and a hook on one side of the bandage cuff. To attach the cuff, simply insert the hook into the bandage strip. Measure the circumference of your arm to make sure that it lies within the range indicated on the cuff. The following cuff sizes are available for all models: Adult arm circumference: 24 - 32 cm

Cotton hook cuffs On one side of these cuffs, metal bars have been worked into the fabric covering; metal hooks have been riveted to the fabric on the other side.

The metal hooks are inserted into the metal bars in the cuff fabric. Measure the circumference of your arm to make sure that it lies within the range indicated on the cuff. The following cuff sizes are available for all models: Adult arm circumference: 24 - 32 cm
How to put on the cuffs A.
Nylon-velcro cuffs Place the cuff 2-3 cm above the bend of the elbow on the naked left upper arm; make sure that the symbol 0 is above the artery. Close the cuff by means of the velcro closure.

After you have put on the cuff, pump it up by pressing the bulb until the pressure in the cuff is about 20 mm Hg above the expected systolic blood pressure (upper value). It is easy to read the values by comparing the column of mercury, which rises due to pumping, with the graduated scale. It is very important that the person whose blood pressure is being taken is relaxed and sitting comfortably. Furthermore, the lower arm should be placed at the level of the heart with the inside of the arm facing upwards. Place the chestpiece of the stethoscope - preferably our anestophon® model (Cat. No. 4177-01 - 4177-05) - on the artery under the cuff. To be able to measure blood pressure, open the air-release screw on the bulb by turning it counter-clockwise. Ideally, the rate of air release should be 2 - 3 mm Hg/s. This rate can be adjusted by turning the screw very carefully. Perform a visual check of the air-release rate by looking at the scale: The mercury display must move along the scale at a speed of 1 - 1.5 graduations per second. Once the upper blood pressure value (i.e. systolic pressure) has been reached, a rhythmic thurnping sound is heard. The systolic pressure, or upper blood pressure value, is the pressure measured when the heart contracts, forcing blood into the blood vessels. When the lower blood pressure value (diastolic pressure) is reached, the thumping sound stops. The diastolic pressure, or lower blood pressure value, is the pressure measured when the cardiac muscle is expanded and fills up with blood again. Now open the air-release valve completely so that air can escape from the cuff immediately. The blood pressure measurement procedure has now been completed. We expressly call your attention to the fact that only a physician is capable of precisely analyzing the blood pressure readings you have taken. We recommend locking the unit after every measurement and any time the unit is transported. To do this, turn the unit on its side carefully until no more mercury is visible. Then close the mercury lock.

Manometer and bulb Cleaning Wipe off the manometer and bulb with a damp cloth. Disinfection is not necessary as the parts do not come into direct contact with the patient's body. Cuffs Cleaning: After removing the bladder, wipe the nylon-velcro covers with a damp cloth. Alternatively, these can be washed with soap and cold water like all the other cuffs. If you decide on the latter course, rinse the cuffs with clear water afterwards and let them air dry. Wipe the bladder and tubes with a damp cloth. Disinfection: After removing the bladder, wash the cuff covers in cold water to which disinfectant has been added. Afterwards, let them air dry. The bladder and tubes can be wiped with a cotton cloth moistened with ethanol.
IMPORTANT! Do not iron nylon-velcro cuffs! Never expose the cuffs to intensive solar radiation! Never touch the cuff fabric or latex parts with a sharp instrument, as this could cause damage! 3. Glass tube Cleaning To ensure exact measurements, the glass tube should be cleaned at regular intervals (e.g. in accordance with the recommended timetable for maintenance) with the wire cleaner included. Cleaning makes it possible for the mercury to move up and down in the glass freely and thus react quickly to changes in cuff pressure. Take care to avoid getting any mercury on your clothing during cleaning. Any material that is contaminated with mercury should be sealed in a container in a plastic bag prior to disposal. Replacement Wall models must be removed from their mounts carefully before replacing the glass tube. A. diplomat-presameter®, nova-presameter® models, nova-presameter® wall model Gently tilt your unit on its left side and wait until no more mercury can be seen. Close the mercury lock. Lift the sliding piece at the lower end of the glass tube (see Fig. 3) and remove the glass tube. Now insert the cleaned or new glass tube precisely in lower and upper holders. Please observe the notice "Important information on dealing with mercury". B. nova-presameter®, global de luxe® models Gently tilt your unit backwards and wait until no more mercury can be seen. Close the mercury lock. Instead of the sliding piece, the ring must be lifted carefully (see Fig. 4). Insert the cleaned or new glass tube precisely in lower and upper holders. C. empire® N desk, wall, stand and anaesthetic models See "B". Instead of the ring, the raised plastic part must be carefully pulled up (see Fig. 5).

Important information on dealing with mercury 1.
Guidelines and precautionary regulations Mercurial sphygmomanometers should be handled carefully. Take care not to drop the unit or knock it to the floor and handle the unit gently to avoid damaging it. To ensure that no mercury escapes from your unit, you should send it in to us or bring it to your specialized dealer for periodic inspection.

Health and safety issues when dealing with mercury Exposure to mercury can have grave toxicological effects. Adsorption of mercury leads to neuro-pyschiatric disturbances and, in extreme cases, kidney disease. For this reason, precautions should be taken when servicing mercurial sphygmomanometers. Units that are being cleaned and repaired should be placed on a smooth, impermeable tablet that is tilted away from the operator at a horizontal angle of 100°C and has a waterfilled depression. Wear suitable gloves (e.g. latex gloves) to avoid direct skin contact. Ensure that your work area is well ventilated and avoid coming into contact with or breathing vapors. A high level of workplace hygiene is imperative in areas in which mercurial apparatus is repaired. It is known that persons who repair mercurial apparatus are subjected to chronic mercury exposure.
3. Rules for dealing with spilled mercury 3.1 Always wear rubber gloves when dealing with spilled mercury. 3.2 Avoid breathing in the mercury and never use an open vacuum system to collect the mercury. 3.3 Bring all small droplets of the spilled mercury together to form one clot and immediately deposit all mercury in a tightly sealed container. 3.4 After mechanically removing the mercury, the contaminated surface should be treated with cleaning agent. The agent, which is composed of equal parts of calcium hydroxide and powdered sulfur, must be mixed with water until it forms a thin paste. 3.5 Apply the paste to the contaminated surfaces and allow it to dry. After 24 hours, wipe away the paste with lukewarm water. Always ensure that the affected area is well ventilated.
Disposal information Mercury must be recycled or disposed of as waste requiring special monitoring.
Test of precision Remove the tube from the manometer. Open the mercury lock. The mercury column should come to rest at "0". If the mercury column comes to rest below the 0, mercury must be added. In this case, either take it to an authorized Riester dealer or send it to us for recalibration.
Monitoring of instruments EU countries except Germany: Legal regulations for monitoring instruments apply to all EU countries except Germany. Countries outside the EU: For any countries where no legal regulations exist for monitoring instruments, it is recommended to examine the accuracy of measuring instruments in 2-year intervals.
